How can I file a HIPPA complaint?

When you receive medical care in Louisiana, you have probably been asked to sign a document related to HIPPA. HIPPA is a law that protects your medical information. It set requirements for how such information is shared and stored. If you feel there has been a violation of HIPPA, you have the right to report it.

The U.S. Department of Health & Human Services explains any complaint you file in regards to HIPPA rights violations must be done within 180 days from when you became aware of it. You must also file it using fax, mail, online or email and the name of the entity involved must be given. You should explain what happened and why you believe your rights were violated.

Filing by mail, fax or email requires filling out the Health Information Privacy Complaint Form Package. This is downloaded as a pdf. You also have the option of not using the form and simply writing out your complaint if you do not have access to a computer. You must include your name and contact information, the contact information for the entity you are reporting on and an explanation of what happened. You must sign it and date it. 

If you want to file online, you must use the OCR Complaint Portal. It will lead you through the process. This is fairly easy to do if you have access to a computer with internet. 

Remember to file a complaint you must provide your name and contact information, including your phone number. If you do not provide this information, your complaint may not be reviewed. This information is for education and is not legal advice.
